The Pre-Intake Worker will then schedule you for an appointment for an Intake Assessment. Depending
on the urgency of the situation, it may be between 2 days and 3 weeks to get
this appointment.
Once your appointment is scheduled (and as time permits), you will receive
an introductory letter from us in the mail. This letter will tell you
what you need to bring to the first appointment (such as identification,
proof of income), and will also include a questionnaire to be filled out
right before your appointment.
On the day of the Intake Assessment, be prepared to stay from 1½ - 2 hours. You will first
meet briefly with the Billing Manager; the remainder of time will be spent in your
Intake with the Clinician. During that time, the Clinician will ask you more in-depth
questions about the issues that brought you to the agency for services as well as information
about your history.
Payment and Schedule of Fees:
UCPC is a private, non-profit service organization. The agency accepts Medicaid, Medicare,
and some private insurance. For individuals without medical insurance, the agency provides
services based on a sliding scale fee. This fee is based on an individual's ability to pay.
No person is ever denied services because of an inability to pay.
For specific questions about our billing policies or scale, you may contact Keisha Caroll,
our Billing Manager, at 908.756.6870.
